As a Registered Massage Therapist, my treatments are covered by most extended healthcare benefits packages. An insurance receipt is provided at the end of your treatment. Please note you are responsible for obtaining the details of your insurance plan.
Policies
Privacy Policy
At the time of your first appointment, you will be required to complete a confidential health history form. This form must be updated once a year or any time there are significant changes to your health history (ex. surgery, major illness). The information in your file can only be released with your consent or as required by law. In order to best serve you, I may discuss your case with other healthcare providers.
Cancellation Policy
I have a strict 24-hour cancellation policy. If you cancel or reschedule your appointment with less than 24 hours’ notice, for any reason, you will be charged the full treatment cost.
I understand that life happens. However, as a sole practitioner, 100% of my income comes from my appointments with valued patients like you. When you cancel on short notice, my livelihood is jeopardized. You are also taking an appointment away from someone who could otherwise benefit from my care.
Need to cancel on short notice? Donate your appointment to a friend or a family member! It’s win-win: They get a fabulous massage while you save the cancellation fee.
Please note that insurance providers do not cover missed appointments. Future appointments cannot be scheduled until any outstanding invoice is paid. Thank you for valuing my time.
Late Arrival Policy
If you arrive late for your appointment for any reason, you will forfeit the elapsed time and will be charged the full treatment cost.
Appointment Times
Please note that the time you have booked is the length of your appointment, not the hands-on time. At your first appointment, I conduct a thorough intake interview where I cover your health history and your reasons for seeking massage therapy. I provide a detailed description of my treatment plan so there are never any surprises during your massage. I obtain verbal and written consent for your treatment. Orthopedic assessments may be required to provide a safe and effective treatment. I do my best to maximize your hands-on time, but please be aware that it is rarely the full length of the appointment.
Refusal of Service
For safety purposes, I reserve the right to refuse or discontinue treatment at any time. Reasons may include, but are not limited to, the following:
- Intoxication
- Derogatory or bigoted comments
- Sexual or romantic advances
- Illness or infectious conditions
- Contagious or unidentified skin conditions (ex. rashes, warts, mysterious bug bites)
If you are sick, please reschedule your appointment. If you develop a skin condition, please clear it with your doctor before receiving a massage.
